Submit a grant proposal

All of the following information below needs to be included in your submission in order to be considered for a grant. We welcome you to use your own form or put items in a different order. Please be sure you've included all the requested information.

Please note, the Martin Family Foundation DOES NOT accept unsolicited grant requests.

  1. A thorough description of your nonprofit's Mission and its scope of work.

  2. A description of your proposed project along with the project's timeline.*

  3. The amount of funds requested.

  4. A description of the evaluation process for your project.

  5. The budget for the proposed project, including the amount of donations already received and

    anticipated donations in the coming year. *

  6. Please include the percentage of funding your organization received in the last year from

    corporations, government sources, nonprofit organizations, and individuals.

  7. Your nonprofit’s annual budget for the last two years.

  8. Your nonprofit’s Profit & Loss statements for the last two fiscal years.

  9. There is no need to send your Nonprofit’s tax returns or audited statements.

  10. Your organization’s 501-C (3) designation letter (if you haven’t done so as of yet.)
