Submit a grant proposal
All of the following information below needs to be included in your submission in order to be considered for a grant. We welcome you to use your own form or put items in a different order. Please be sure you've included all the requested information.
Please note, the Martin Family Foundation DOES NOT accept unsolicited grant requests.
- A thorough description of your nonprofit's Mission and its scope of work. 
- A description of your proposed project along with the project's timeline.* 
- The amount of funds requested. 
- A description of the evaluation process for your project. 
- The budget for the proposed project, including the amount of donations already received and - anticipated donations in the coming year. * 
- Please include the percentage of funding your organization received in the last year from - corporations, government sources, nonprofit organizations, and individuals. 
- Your nonprofit’s annual budget for the last two years. 
- Your nonprofit’s Profit & Loss statements for the last two fiscal years. 
- There is no need to send your Nonprofit’s tax returns or audited statements. 
- Your organization’s 501-C (3) designation letter (if you haven’t done so as of yet.)
